To request any TASIS Dorado Official Documents (transcripts, diplomas, and other official documents) to graduating or departing students and for the End of Academic Year, please download the form by grade and fill it accordingly, please follow all instructions carefully and submit to the Business Office once the form is completed

Alumni Transcript Request Online Form 
One Alumni Transcript is $5.00. Each additional copy ordered at the same time is $2.00.

ALUMNI: Orders with incomplete information and/or insufficient payment are NOT processed.

• Online Payment available by PayPal accepted on this form.
• Payments by Cash, ATH, Check or Money Order must be made at the TASIS Dorado Business Office
• Credit Cards (MasterCard, Visa, AMEX) may pay by phone by calling Gloria Santiago at 787-796-0440 Ext. 294.
